The Physical Therapy evaluates each patient and provides medically prescribed treatments and education to patients to restore, improve, or maintain the patient’s optimal level of functioning, independence, and quality of life.  The Physical Therapist also supervises Physical Therapist Assistants in carrying out the prescribed treatment plan.

Overview of Responsibilities

  • Evaluates each patient utilizing applicable diagnostic, prognostic, and functional ability tests.
  • Re-evaluates patient's status at appropriate intervals.
  • Interprets evaluation data, history, and applicable medical documentation to determine rehabilitation potential and potential benefit from intervention.
  • Identifies physical, social, and psychological needs of the patient, family, and significant other.
  • Sets and documents rehabilitation goals.
  • Manages treatment planning
  • Provides patient education appropriate to each patient.
  • Collaborates with patient, caregivers and other healthcare providers in planning discharge from rehabilitation services.

Education/Experience

Education: BS, MS or DPT Degree or Certification in Physical Therapy, a Bachelor’s, Master’s, or Doctoral degree in Physical Therapy from an ACTA (accredited educational program). 

LICENSURE/CERTIFICATION: Must be fully licensed in Indiana to practice as a Physical Therapist.  May work with a Temporary Permit under the stipulated supervision of a PT in accordance with Indiana regulations. Must maintain active CPR certification.

Schedule Details

Day Shift. 40 hours weekly. 7:30a-6:00p.
