Under the supervision of the Reid Health Transportation Supervisor, is responsible for transporting and escorting patients and participants between their homes, Reid Health, Reid Health PACE Center, and other designated locations.

Provide regular and dependable attendance.
Drive a specially equipped van to deliver persons to varied destinations. Assure that wheelchair lifts and tie-down equipment are properly operated and that participants/patients are delivered to appointments in a timely manner.
Ensure cleanliness and functioning of vehicle and equipment. Inspect van and maintain it in a clear, orderly, and safe condition. Report the need for maintenance.
Follow established schedules, adjust schedules to meet unanticipated changes, and determine the safest and most efficient pick-up and delivery routes.
Assist passengers on and off the van in a safe and courteous manner. Handle medical equipment and any emergencies that may arise.
Complete documentation as required.
Ask for assistance when needed.
Attend and successfully execute required annual driver training courses and participate in mandatory in-service training.
Be accountable for scope of work and compliance with company policies and procedures.
Follow all Policies and Procedures and OSHA safety guidelines.
Maintain the confidentiality of all company procedures and information about participants, clients, or families.
Handle potentially infectious specimens with appropriate biohazard precautions, and practice standard precautions.
Participate in and support Quality Improvement initiatives.
Participate in continuing education classes and any required staff and training meetings. Maintain professional affiliations and any required certifications.
Perform other duties as required or requested.
Comply with CMS PACE regulations as they pertain to this role.
Perform other duties as assigned.
Education Required: High School diploma
Education Preferred: High School diploma
Experience Required: 1 year working with the frail and elderly population
Experience Preferred: 2 years in a transportation and/or patient transport setting
Licensure and Certification
Licensure: Valid driver’s license
Certifications: CPR
Age Requirement Minimum: 21 years of age
Age Requirement Preferred: 25 years of age or older
