Under the direction and supervision of the Home Care Coordinator and Clinical Manager, assists with participant medication management and implementation of nursing services in the home setting as needed for the Reid Health PACE program. This individual works hand in hand with PACE care providers to achieve quality outcomes for the participant. Communicates participant’s condition to primary caregivers, providers, and the interdisciplinary team. Accepts other duties as assigned by the Clinical Manager and within the scope of practice.

1. Provide skilled nursing care in the home environment under the direction of a Provider.
2. Participates in the development and revision of the participant’s plan of care as a member of the IDT.
3. Coordinates and supervised all nursing care for Reid Health PACE participants in the nursing home/boarding home according to program standards. Includes interaction with other facility staff and taking part of the Home Safety Assessment.
4. Provide basic medical care, including wound care, medication management and collecting specimens as directed by the Provider.
5. Accesses home care services system, makes recommendations and offers solutions to revise or modify systems to better meet participant needs.
6. Actively participates in Interdisciplinary Team Meetings.
7. Assists with pre-enrollment in-home assessments.
8. Instructs and educates participants and family on medication and treatment instructions.
9. Monitor and reports changes in patient’s condition to the provider.
10. Ensures chart documentation is completed in a timely manner and in accordance with practice policy and procedure.
11. Performs lab tests in accordance with needs of participant and as directed by the Provider. Prepares specimens for in-house and reference labs as appropriate.
12. Observes, records, and reports participant’s condition and reaction to drugs and treatments to Providers.
13. Must be committed to Participant Satisfaction and Participant Safety.
14. Maintains participants’ medical record and fulfills Reid Health PACE charting and reporting requirements.
15. Responds to and performs triage of incoming telephone calls. May be assigned to Center’s Triage. Provision of emergency care as needed.
16. Participates in administering participant vaccinations. Documents and records appropriately.
17. Prepares equipment and aids providers during treatment, examination, and testing of participants.
18. Assists other staff in the home setting or clinic during appointments and emergencies.
19. Maintains supply inventory for clinic medication room.
20. Effectively communicates in clinical meetings or any other meetings as assigned.
21. Provides support to participants across all care settings.
22. Monitors operational compliance with OSHA regulations, Infection Control protocol, and agency policies and procedures.
23. Participates in and supports Quality Improvement initiatives.
24. Participates in continuing education classes, staff meetings and any required trainings. Maintains professional affiliations and any required certifications.
25. Complies with CMS PACE regulations as they pertain to this role.
26. Participate in family meetings, staff meetings, in-service and training programs, staff orientation and education activities, rehab services meetings and Quality Improvement activities.
27. Provides on-call coverage on a rotating basis with other nursing staff to triage, advise, and coordinate participant care within scope of practice.
28. Performs other duties as required or requested.
• Education Required: ASN
• Education Preferred: BSN
• Experience Required: A minimum of one year experience working with the frail or elderly population.
• Experience Preferred: 3+years of experience working in health care facility or practice.
Licensure and Certification
• Licensure: License and current registration to practice as a registered nurse in Indiana
• Certifications: BLS
