Contacts insurance to obtain insurance authorization of potential patients. Overview of Responsibilities

  1. The primary customers are patients and clients and the mission is to build long-term relationships with client institutions.
    A. Participate in continuing education/professional development activities.
    B. Completes rehab assessment within 72 hours of admission, excluding holidays and weekends.
    C. Communicate and demonstrate a professional image/attitude for patients, families, clients, coworkers, and others.
    D. Participate in pre-admission screening.
    E. Develop professional and community contact plan in consultation with program director.
    F. Maintain effective communication with patients, families, referral sources, clients, and team members.
    G. Collaborates with other hospital programs and community agencies with which patient and families are involved.
    H. Computer knowledge of Hospital information systems, and utilizes same in daily caseload.
    I. Participate in budgeting process for professional and community relations activities.

  2. The primary customers are team members and the mission is to provide the highest quality of rehabilitation services.

  3. Utilizes effective communication skills through interpersonal approach with patient, family, and other members of the health care team, respecting individual differences.
    A. Reports information accurately, concisely, and in a timely manner, to facilitate treatment and provision of services.

  4. The primary customers are patients and the mission is to provide the highest quality of rehabilitation services.
    A. Participate in unit quality management activities.
    B. Collect quality review data.
    C. Keeps accurate record of contacts and services rendered using the permanent medical record.
    D. Collaborates with nursing to see that social and medical information which accompanies the transferred patient is completed to ensure continuity of care.

Education/Experience

The incumbent shall, at a minimum, be a licensed or certified clinician appropriately trained to assess the patient medically and functionally.   A track record of successful professional and community relations experience in a health care setting, strong interpersonal skills and experience working with an adult geriatric disabled population is desirable.  Provision of own transportation is necessary.

Schedule Details

Day shift. 40 hours weekly. 8:00am - 4:30pm.
