Overview of Responsibilities

The Licensed Practical Nurse is assigned clinical responsibilities based on teleprovider assignment. In collaboration with and at the direction of the teleprovider, LPN provides care based on identified needs.

    • Demonstrates ability to work independently within scope of practice.
    • Builds relationships with, and uses, appropriate resources – Teleprovider, primary RN, Charge nurse, Unit Director, RHPA Directors, House Supervisor, Pharmacy, etc.
    • Demonstrates superior oral communication skills needed to develop patient rapport, establish connection with telehealth provider, and effectively communicate encounter plan with patient and patient’s family.
    • Demonstrate written communication skills necessary for charting and documentation within electronic health record.
    • Demonstrates ability in taking patient vitals.
    • Documentation portrays the patient’s condition.
    • Documents changes in patient condition and responses to interventions.
    • Documents communications with physicians and /or other members of the healthcare team.
    • Demonstrates competence in utilizations of all EMR and resource systems.
    • Completes medication reconciliation.
    • Demonstrates knowledge and skills necessary to provide care appropriate to the age of patients served.
    • Demonstrates ability to connect on a human level, at a minimum making eye contact with and greeting patients, families, visitors, and staff.
    • Shows courteous interactions, including correct voice inflection and positive body language.
    • Demonstrates empathy by acknowledging and validating patient’s situation or experience (“It must be hard to…” “it must be difficult to…”).
    • Demonstrates compassion by ability to “walk in someone’s shoes” – get to their level, to understand where they are and what they are feeling and help them get to where they want to be.
    • Appropriately uses therapeutic touch to calm and comfort.
    • Shows willingness to take ownership; uses “I” or “we” rather than “they” or “them”. Blame is not directed at others.
    • Assumes accountability for professional growth.
    • Committed to patient satisfaction and patient safety.
    • Approaches customers when they appear to be dissatisfied or in need of other assistance and immediately facilitate service recovery efforts.
    • Responds to call lights within nursing’s goal time frame.
    • Supports methods to keep noise at a minimum.
    • Routinely meets the culture of every patient every time regarding the patient experience by utilizing AIDET & CARE principles and making connections with

Education/Experience

  • Graduate of an accredited Practical nursing program
  • Previous experience in medical office or inpatient hospital preferred.

Schedule Details

40 hours weekly. Onsite.
Monday-Thursday 10-hour shifts
Friday-Sunday 12-hour shifts

Schedule will be dependent upon need once service is implemented.
