1. Works proficiently in all information systems relating to Pharmacy
2. Reviews and interprets physician orders/prescriptions and performs clinical checking such as those related to interactions or allergy contraindications
3. Checks all medications and IV’s before being dispensed from the Pharmacy
4. Participates in concurrent monitoring of drug therapy and quality improvement programs to ensure appropriate, safe, and cost-effective medication therapy
5. Performs clinical activities such as pharmacokinetic consults, renal dosage adjustments, and IV to PO conversions according to protocol.
6. Checks outpatient prescriptions. Counsels each outpatient who requests counseling by a pharmacist and other outpatients as time allows
7. Documents clinical interventions through Pharmacy OneSource
8. Prepares medications and provides drug information during Code Blue
9. Participates in the supervision of pharmacy externs
10. Participates in orienting new technicians and pharmacists
11. Performs accurate, thoughtful technician evaluations on each technician supervised
12. Provides shift to shift communication at the end of each work shift
13. Counsels patients on medications, such discharge counseling or Coumadin
14. Participates in Adverse Drug Reaction review and documentation
15. Attends and contributes to pharmacy staff meetings and in-services
16. Provides drug information to health care professionals and patients
17. Demonstrates an awareness of fire/safety hazards by keeping current with the guidelines
18. Participates in QA&I program.
19. Participates in employee health infection control requirements
20. Demonstrates competency in providing pharmaceutical services to pediatric, adult, and geriatric patients
Education Required: Graduate of an accredited School of Pharmacy
Education Preferred: As above
Experience Required and Experience Preferred: Hospital Pharmacy experience preferred but not required.
Licensure: Active pharmacist license in the State of Indiana
Certifications: None

Work Environment
Mix of sedentary work in the Pharmacy on the Reid Health Main Campus and standing in the Outpatient Pharmacy. Some pharmacists also work in a Satellite on a Nursing Unit.
