Assesses patient needs related to activities and leisure. Plans, organizes and conducts individual or group therapeutic activity programs designed to improve the patient’s physical, mental, and behavioral well being, for patients in Psychiatric Services.

Flexible scheduling. Monday through Friday, 8 hour days.

• Meets with each patient to assess the patient’s needs and establish rapport.
• Develops individualized treatment plan for each patient related to activity therapy needs.
• Participates in multi-disciplinary team meetings to develop comprehensive treatment plan.
• Plans and implements patient activity and education groups designed to meet patient’s individual needs and to enhance patient’s general quality of life.
• Reviews and updates treatment plans and therapy groups as needed.
• Participates in discharge planning process.
• Enters daily charges via the SMS.
• Implements activity therapy interventions from treatment plan.
• Conducts activity therapy, education groups designed to meet treatment plan goals, and to enhance patient’s adjustment and quality of life.
• Works with team members under the supervision of the Charge RN in implementation of groups and activities with individual patients.
• Evaluates activity treatment plan on an ongoing basis updating as the patient’s needs dictate.
• Documentation includes Master Treatment Plan, Activity Therapy Assessment, group notes and Snoezelen treatment notes.
• Snoezelen treatment – outpatient sessions/inpatient sessions.

Education Required:  Graduate from an accredited college with Bachelors degree or major in Mental Health or related field

Experience required: Experience working with mentally ill, developmentally disabled, memory care or corrections population in an education/group activities setting required
